1. The Ignored Ice Warnings Before the Titanic Sank

Multiple ships sent ice warnings to the Titanic hours before it hit an iceberg. Radio operators, overwhelmed with passenger messages, never passed them on to the bridge. It was a warning buried beneath chatter, and it cost over 1,500 lives. 

2. The Scientist Who Warned About Hydrogen in the Hindenburg

Aerospace expert Hugo Eckener cautioned against using hydrogen in airships due to its extreme flammability. His warnings were dismissed to save costs. The Hindenburg went up in flames in 1937, proving him right in the worst way.

David A. Johnston, a USGS geologist, issued repeated warnings before Mount St. Helens erupted. He pushed to keep people away, but his concerns were downplayed. He died in the eruption he predicted. 

4. The Repeated Warnings Before the Challenger Launch

Engineers at Morton Thiokol pleaded to delay the Challenger launch due to freezing temperatures. NASA overruled them under schedule pressure. The shuttle exploded 73 seconds after takeoff, killing all aboard.

Valery Legasov identified serious flaws in the RBMK reactor design years before the Chernobyl meltdown. Soviet authorities ignored his warnings. He was one of the first to respond and later took his own life out of frustration.
